HOW DO I...
Register With The Practice ?

If you wish to register with the surgery, you will need to bring in your medical card with your NHS number on it (this is different to your National Insurance number). If you cannot find your medical card you can fill in a form at the surgery although this is more complicated. You will always be offered an appointment for a New Patient Medical check up.

We suggest that you have a chat with Denise Poole the office manager before registering and she will explain about the running of the practice. If you wish to have a chat with one of the doctors beforehand this can also be arranged.

Change My Details ?

Please let us know your new address as soon as possible if you move so that we can keep our records up to date. Please check with reception that your new address is still within our practice area as we cannot register anyone who lives too far away to visit. If your new address lies outside this area, we will unfortunately have to ask you to find a new doctor closer to your new home. Please also tell us if you change your phone number (eg if you go on to cable).

This is very important in case we need to contact you. If you are in any doubt please contact our office manager

Obtain Telephone Advice ?

To arrange a telephone consultation please contact the surgery before 11.30am. You will be given a time to call back when the doctor will be free to speak to you.

Make An Appointment ?

We operate an appointment system for the efficient running of the practice. Routine appointments should be booked in advance. Urgent cases will be offered an appointment the same day although you may not be able to see your usual doctor. If you feel your request is urgent please telephone the surgery at 8.30 am. If an urgent appointment is inappropriate for the problem you present with or you have more than one problem, the doctor may ask you to book a routine appointment.

Please book one appointment for each person. If you think your problem may require extra time, please ask for a double appointment.

If you wish to see a particular doctor then appointments may need to be booked well in advance for routine matters.

If you are unable to keep an appointment, please contact the surgery giving as much time as you can so that the appointment can be given to someone else.

HOW DO I...
Obtain Home Visits ?

Patients too ill or disabled to come to the surgery can be visited at home although this visit may not always be by your usual doctor. Except in an emergency, requests for visits should be made by 10.30am. You may be asked for some information so that the doctor can assess the priority. It is usually better however to attend the surgery where facilities are better. Sometimes it is helpful to speak to the doctor personally on the telephone.

Obtain Test Results ?

Laboratory test results can be obtained by ringing reception after 2.30pm. Any patient with test results requiring action will normally be contacted by the practice. Results will only be given to the patient concerned or the parent or guardian of young children.
