Obtain Repeat Prescriptions ?
These may be obtained by handing in or posting your prescription counterfoil with
any requested items ticked. If you are handing in your request at the surgery please place it in either of the red boxes at reception.
Please allow 48 hours for the request to be processed. If you prefer, you may enclose an SAE for it to be returned by post.
Do not worry if you lose your counterfoil as our receptionists can normally help.
From time to time the doctor may ask you to come to the surgery to discuss your ongoing
treatment. We do not accept requests for prescriptions over the telephone, unless you are housebound or the doctor has agreed. This
is due to the obvious potential for error. We are happy to accept requests by
fax on (0121) 733 6892.
HOW DO I....
Obtain Sick Notes ?
For all illnesses lasting seven days or less, a sick note from your doctor is
not required. Instead, on returning to work, complete a self certification form
which is available from your employer. If a note for a private insurance scheme
is required then a charge will be made as this falls outside the NHS system.
